Where is my center located?
We’re located in Sunnyvale West, just 10 minutes from Sunnyvale MP3 via CA-237. The entrance to the building is on Potrero Ave.

The pharmacy stays open until 7 p.m.
Radiology is closed daily for lunch from 12:30 – 1:30 p.m.

If you’re feeling under the weather with a sore throat, cough, fever or other cold symptoms, there’s a virtual primary care provider available to see you any time of the day (or night). Jazziel is a board-certified nurse practitioner with nine years of experience, plus an additional eight years as a registered nurse. She’s dedicated to meeting the growing need for high-quality primary care and helping members manage acute and ongoing conditions so they can maintain a healthy lifestyle.
Specialties: Primary care, family medicine, pediatrics, adult and geriatric care, palliative care, community health, acute and chronic condition management and preventive care

Julia is a nurse practitioner with nearly 20 years of healthcare experience, including five years in nursing and clinical roles. She’s committed to delivering compassionate, well-rounded care and supporting members through both acute needs and long-term wellness goals.
Specialties: Adult primary care, pediatric care, women’s health, urgent care, acute and long-term condition support, wound care and preventive care
