We’re located in Sunnyvale West, just 10 minutes from Sunnyvale MP3 via CA-237. The entrance to the building is on Potrero Ave.

The pharmacy stays open until 7 p.m.
Radiology is closed daily for lunch from 12:30 – 1:30 p.m.

MD
Catherine has been in the psychiatry field for two decades in various settings, from her own private practice to psychiatric emergency rooms and inpatient hospitals. She’s also taught at Stanford University and has served as a community guest on NPR’s California Report Magazine.
Specialties: Collaborating with members and their care teams for a holistic and comprehensive approach to their behavioral health.
Hobbies/Interests: Jogging, writing and reading, drawing and painting, spending time with loved ones and baking specialty cakes for her kids.

Psy.D
Josh holds a doctorate in clinical psychology from the California School of Professional Psychology and has 14 years of experience practicing in hospitals, onsite health centers, outpatient psychiatric clinics and private practices.

Hobbies/Interests: Playing music, spending time with family, eating and exploring

Quang Pham,
PT, DPT
Quang holds a doctorate degree in physical therapy from Columbia University and is a certified strength and conditioning specialist. He has worked in a variety of settings including acute care, outpatient, and home health.
Specialties: General post-op recovery, orthopedic injuries, rehab and conditioning related to sports specializing in treatment of runners, power and Olympic weightlifters, and golfers.
Hobbies/Interests: Photography and videography, golf, tennis, powerlifting and hanging out with his two cats, Zuko and Mumu

Daniel Chen,
PT, DPT
In addition to his doctorate in physical therapy, Daniel has also completed certification for strength and conditioning with experience in a variety of settings, including private practice and large hospitals.
Specialty: Post-surgical recovery, and racket sports such as tennis, pickleball, and badminton.
Hobbies/Interests: Tennis, watching the NBA and Duke University basketball, trying new restaurants and spending time with friends
